چکیده
1 of 2 DESCRIPTION A 78-year-old male presented with jaundice, fever, rigors and right upper quadrant pain. Ultrasound demonstrated gallbladder empyema and common bile duct (CBD) stones with mild CBD dilatation. Medical morbidity and prior partial gastrectomy precluded endoscopic treatment. Under ultrasound guidance an 8fr Navarre pigtail drain (Bard Limited, UK) was inserted into the gall bladder and left insitu for 4 weeks. After 4 weeks, an 8fr Brite tip vascular sheath (Cordis-Europa, Netherlands) was introduced over a guidewire into the gallbladder via the cholecystostomy tract. A 5fr hydrophilic catheter (Cobra C2) and 0.035 guidewire (Terumo, Belgium) was used to traverse the cystic duct and CBD into the duodenum ( fi gure 1A,B ). The 0.035 wire was exchanged for a 0.018 VI8 Control Wire (Boston-Scientifi c, Natick, Massachusetts, USA) and an 8 mm peripheral cutting balloon was used to perform the papillotomy ( fi gure 1C ). Following papillotomy an 8 mm Images in...

Percutaneous trans-papillary elimination of common bile duct stones using an existing gallbladder drain for access.
BACKGROUND The presence of stones in the common bile duct (CBD) may cause complications such as obstructing jaundice or ascending cholangitis, and the stones should be removed. OBJECTIVES To assess the efficacy of percutaneous elimination of CBD stones from the gallbladder through the papilla. METHODS During a 4 year period, six patients (five men and one woman, mean age 71.5 years) who had...
متن کاملEndoscopic sphincteroplasty: a novel and safe alternative to papillotomy in the management of bile duct stones.
Removal of bile duct stones during endoscopic retrograde cholangiopancreatography (ERCP) usually includes papillotomy. Papillotomy is associated with occasional complications and in addition, the longterm sequelae of papillotomy in young patients having laparoscopic cholecystectomy remain unclear. As an alternative to papillotomy, this study prospectively evaluated the efficacy and safety of en...
متن کاملA 10 year single centre experience of percutaneous and endoscopic extraction of bile duct stones with T tube in situ.
A non-randomised single centre study of 226 consecutive patients referred over 10 years with retained common bile duct stones and a T tube in situ or a cholecystostomy tube are reported. Percutaneous extraction was attempted in 204 and endoscopic extraction in 68 patients. Percutaneous clearance was achieved in 158 (77.5%) patients and endoscopic clearance in 52 (76.5%) patients. Six of 153 (3....
متن کاملPercutaneous Papillary Large Balloon Dilation during Percutaneous Cholangioscopic Lithotripsy for the Treatment of Large Bile-Duct Stones: A Feasibility Study
When access to a major duodenal papilla or endoscopic retrograde cholangiopancreatography has failed, percutaneous transhepatic cholangioscopic lithotripsy (PTCS-L) may be useful for removing common bile duct (CBD) stones. However, the feasibility and usefulness of percutaneous transhepatic papillary large-balloon dilation (PPLBD) during PTCS-L for the removal of large CBD stones has not been e...
متن کاملPercutaneous Treatment of Extrahepatic Bile Duct Stones Assisted by Balloon Sphincteroplasty and Occlusion Balloon
OBJECTIVE To describe the technical feasibility and usefulness of extrahepatic biliary stone removal by balloon sphincteroplasty and occlusion balloon pushing. MATERIALS AND METHODS Fifteen patients with extrahepatic bile duct stones were included in this study. Endoscopic stone removal was not successful in 13 patients, and two patients refused the procedure due to endoscopy phobia. At first...
